Scheduling and Cancellations:

Our staff’s flexible schedule accommodates most current and new patient needs. As a courtesy, if you are not able to keep your appointment please cancel to allow the opportunity for another patient to see the doctor. What to bring to your appointment:

  • A list of any medications you are taking
  • Your Insurance Card
  • Your picture ID card (Driver’s License, etc.)
  • Any relevant past medical records
  • If necessary, a referral from your primary care physician

When to arrive:

For new patients, we schedule your appointment 10 minutes in advance to allow adequate time for you to complete any necessary paperwork. If you would like to complete the paperwork prior to your visit, please click here to download our patient forms. Your initial visit may require up to 60 minutes, depending upon the nature of your problem.
